Our children and families service provides support from the time of diagnosis, right through to aged 18. We provide emotional support, 1:1 family goal development, events and activities throughout the year where families get the chance to meet up with other families, forming friendship and peer support. We provide support using a holistic model of person centred care, alongside our Social Work colleagues our Multi Disciplinary Deaf Specialist Team, and our Explore Mentor programme. Our Social Work team support with rights and entitlements, emotional support and guidance for children and adults. Our MDT team referral pathway is through CDNT and Primary Care. Our Explore Mentor programme provides support for aged 16-25 with support ranging form career guidance, CV preparation and mentoring young adults through the school leaving process through to employment.
Chime’s family support team will provide parents and families with practical information and the emotional support they need to make informed decisions throughout your child’s development, from first diagnosis of hearing loss or deafness to navigating school and college. Chime understands that every family and child is unique with different needs and we aim to work closely with families on their wishes and goals. Chimes new Children and Family Support service will have a person centred ethos with one to one support facilitated by the Family Support Workers. Family and Person Centred Plans will be the mechanism by which the service will be facilitated.
